Complete the form and mail or fax it to the location on the form with your payment. Please don't
register online AND mail or fax in the form. It will result in double billing.
Payment of the Technical Session registration fee entitles the registrant
to entrance to all Monday-Wednesday, Technical Sessions, the Exhibit Hall, Monday Welcome
Reception, Tuesday Conference Reception, and to one copy of the CD ROM of the technical papers.
Single-day registration entitles the registrant to that day's events and one copy of
CD ROM of the technical papers.
AGAIN, NEW THIS YEAR - CD ROMs, not a book, of the technical papers will be included in the
registration fee. Registrants who would like a hard copy of the technical papers need
to purchase the book through Advance Registration by August 29. A limited number of books will be printed and the
conference will not guarantee that copies will be available for sale onsite at the conference.
Technical session registration does not include entrance to the Sunday Educational Sessions or the Sunday Afternoon Presentation Skills Session.
